Upload and track your required documents.
Your employer needs a few details to complete your employment paperwork. This only takes a few minutes, and you'll only need to do this once.
Your employer will fill in your Social Security Number separately — you don't need to enter it here.
Leave any of the four amounts blank if they don't apply to you — that's the same as entering $0.
You choose which acceptable documents to present — your employer can't require a specific one. Pick one option below. Documents must be unexpired. Your employer will still need to look at your original documents to complete Section 2 of the I-9; uploading them here just gets that review started.
You've already submitted onboarding once. Only choose an option below if you need to add or replace documents.
Add every page or part — front and back of a card, or a passport together with its I-94 if your document requires both. Take a photo (tap once per side or page) or attach a photo or PDF.
Add every page or part — front and back of a card, for example. If you're using your driver's license here, add it here too — the copy for your credential file is saved separately.
Take a photo or attach a photo or PDF.
By signing above, you attest under penalty of perjury that the information you've entered is true and correct, and that you are the person named above.
Finish your onboarding form above first — the documents your employer requires are collected there, so there's nothing to upload separately. You'll be able to add other documents here once you submit.
Reviewed documents for your linked staff.
A quick snapshot of where things stand — document status, credentialing readiness, and anything that needs attention first.
Every document on file for your staff, sorted by what's active versus what's still missing.
| Staff member | Document | Issued | Expires | Status | Reviewed | Uploaded | Actions |

Everyone who's missing at least one tracked document — required or optional — sorted by the most outstanding first. This reflects what applies to their specific discipline. Filter below to focus on just required items, just optional ones, or a specific document type.
Available on the Pro plan — see exactly who's missing what, and export a PDF report anytime. Contact Selko to upgrade.
Every license, certification, and credential with a date, sorted by how soon it expires.
Switch between what needs attention soon and the full list — sorted automatically by how close each item is to expiring.

This is what gets pulled into payer credentialing applications automatically — fill it out once, reuse it on every form.
General organization-level credentialing notes — visible to your whole team at a glance.
General files needed for credentialing applications (W-9, blank forms, correspondence) — separate from document compliance tracking. Attach a file to a specific provider from their Notes/Files button in the table below instead, if it's about one person.
Every physical location you see patients from. Mark one as Primary — that's the one payer forms usually ask for first, with additional locations as Secondary/Tertiary.
Organization-level accreditation(s) — not tied to any one staff member.
Reference material for the parts of provider enrollment that happen outside Selko Cred entirely — like Medicare's own PECOS system.
Upload a payer's fillable PDF application, or a bulk roster spreadsheet, map its fields once to your organization/staff data, and generate a filled copy for anyone in seconds — no retyping the same info onto every form.
— or — upload a payer's roster spreadsheet (one row per provider, like Curative's provider roster) instead of a single-provider PDF.
Not sure which row has the headers? Open the file yourself first and count — most payer rosters bury the real header row under several rows of instructions, same as Curative's.
One-click filled payer applications — generated straight from the profile and staff data above. Included on Full Credentialing; contact Selko to upgrade from Basic.
Your staff credentialing data belongs to you — export a complete copy any time, for your own records or to take with you if you ever leave. Covers every staff member's license, NPI, CAQH, and DEA info. Uploaded documents are part of the separate document-tracking service and aren't included here.
Payer-credentialing-level detail on top of basic document compliance — track organizational Tax ID/NPI, accreditations (ACHC, CHAP, Joint Commission), and per-clinician CAQH ID/DEA number, all in one place.

Background and sanctions screening for your staff, plus the audit trail of who's accessed sensitive records and documents.
Checks all your active staff, not only those with an NPI, against OIG's List of Excluded Individuals/Entities (LEIE). OIG publishes a new list by the 10th of every month and Selko loads it for you. A matching NPI, or a matching name and date of birth, is strong. Someone with the same name but a different date of birth is set aside as a different person. A match is a lead to verify, never a finding.
The check runs by itself once a month, on or after this day, but never before the 12th: OIG publishes its new list by the 10th and Selko loads it on the 11th, so the check always uses the new list. Capped at 28 so it always falls on a real date, even in February. Your admins are emailed the result every time.

|---|
Checks your active staff by name against the US Treasury's Specially Designated Nationals (SDN) list, downloaded live from treasury.gov. Unlike the OIG check, there's no ID number to corroborate against — every match here is name-only and needs a human look before acting on it. Each check saves a PDF Audit log (see the history below): who ran it and when, every person checked, and the result. A check that has one can be archived but not deleted.

Checks all your active staff, not only those with an NPI, against SAM.gov's list of people excluded from federal programs (debarred or suspended). Selko refreshes the list from sam.gov every day. A matching NPI is strong. Otherwise it is matched by name, and also by a staff member's former names and aliases, and SAM.gov gives no date of birth, so a name match alone cannot be ruled in or out by Selko. The state and middle initial help. A match is a lead to look at, never a finding. Search the person on sam.gov (Exclusions), and if they are not the one on the list, press Clear this name to keep a record of what you checked.
Also runs by itself once a month, on the day you chose for the OIG check, and emails your admins the result.

The U.S. Department of Justice's free national search across state, territorial and tribal sex offender registries. This one is a manual lookup — Selko can't run it for you or record that you did, so keep your own note of who you searched and when.
Open NSOPW search ↗Every time anyone at your company views or enters a Social Security Number, it's logged here: who, when, and why. Acknowledging an entry marks it reviewed and hides it from the default view. Once an entry has been reviewed, one of your company's own admins can delete it. Unreviewed entries can't be deleted from here (removing a staff member also removes their entries, and that is recorded below too), Selko staff can never delete your entries, and every deletion is itself recorded permanently in the history below.

Each package is a row: name, monthly price, how many staff it covers, and whether it includes the Pro features. Adding a package can also create its Stripe price. Then choose it for a company under that company's subscription settings. To change the price of a package that is already in Stripe, use Stripe prices below.
Change what Stripe charges for each plan. A new price applies to new sign-ups right away. People already subscribed stay on their old price until you move them below. Nothing is prorated.

Downloaded once a day and used for every company's SAM.gov check. A personal SAM.gov API key is allowed 10 requests a day, so each refresh uses one.

Selko support can only open your account to help you if you allow it. You choose whether they can just look or also make changes, and for how long. You can end it at any time. Everything they do is recorded under Compliance > Selko support activity. The one exception: Selko can always see the names and categories of document types your company creates, and how many companies made the same one, to decide what to add to the shared defaults. It never sees which company made them, or any documents or staff.
